What is clinical documentation and what is it for?

Clinical documentation is the set of documents related to each patient’s care processes, identifying the physicians and other professionals involved, with the aim of ensuring the greatest possible integration of each patient’s clinical information, at least within each healthcare center. The primary purpose of the medical record is to facilitate healthcare, recording all data that, under medical criteria, allow truthful and up-to-date knowledge of the patient’s health status.

What information does it ?

Clinical documentation includes all information considered essential for the truthful and updated knowledge of the patient’s health status. This information includes: discharge reports, emergency reports, follow-up reports, complementary tests (blood tests, radiology, pathology, etc.).

How can I request it?

It’s very simple: complete the form available in this web section, and as soon as the documentation is ready, hospital staff will contact you so that you can pick it up.

What documents must I provide when collecting it?

You must remember to bring the following documents to the center when collecting the requested documentation:

If you are the patient:

  • You must bring your original ID (DNI).

If a family member or legal representative will collect it:

  • Authorization signed by the patient stating the patient’s details and the details of the person collecting the documentation.
  • A photocopy of the ID of the person collecting the documentation and a photocopy of the patient’s ID.
  • In the case of a minor, a photocopy of the family record book.
  • If the patient is 14 years old or older, they must sign the authorization mentioned in the first point of this section (Organic Law 3/2018 of December 5 on the Protection of Personal Data and Guarantee of Digital Rights).

If you are requesting documentation for a deceased patient:

  • Photocopy of the Family Record Book / Declaration of heirs / Registry of domestic partnerships.
  • If the patient passed away outside the center where you are making the request, a photocopy of the death certificate or a photocopy of the civil registry removal form.
  • Photocopy of the applicant’s ID.

You can also request your medical record in person at the hospital. You simply need to download the following document, complete it, and submit it to your hospital’s Clinical Documentation Department.
